A form to mail application that integrates a confirmation numbering feature that displays a unique confirmation number for each submission. The number is displayed in the email and the confirmation page. Useful for client contact logs, work orders, intranet communication, etc. (6 KB, ZIP with installation instructions; requires some knowledge of html to configure the standard form)
